is awaiting human approval — fires for tools annotated with
`effect: "write" | "update" | "destructive"` (preferred) or the
legacy `destructive: true` boolean. Clients should render an approval
prompt and POST to `/api/agents/approve` with the matching `approvalId` and
a `decision` of `approve` or `deny`.
